Put down exactly what it asks for. Nothing more, nothing
less. If you are unsure, take the application with you to a
local attorney for a consultation before submitting the
application.

I doubt if it will cost more than a couple of hundred bucks
to get your questions answered in a 45 minute consultation
with an attorney. You need advice from an Illinois attorney
familiar with FOID applications. You will not get that kind
of advice on this board.
