Anyone having an adverse reaction after a SCS implant
needs to report the device to the FDA. The website is
www.fda.gov/medwatch.
Click on Medical Product Reporting then scroll down to
Reporting Form 3500A. The FDA needs this information so
they can evaluate the safety of the product.
