Did you report the incident to your employer right away? I
believe that you are required to do so with work comp
matters. Hopefully, if you did, you have some record of it.

In addition to that (whether you notified the employer in a
timely manner) comes the statute of limitations. That
depends on what state you are in and could be anywhere from
one to six years after the date of accident.
