What is the name of the document I need to fill out and
sign showing that the money I have invested in my new
company ($10,000) is a loan to my new online business, so
I can get it back some day when the business starts making
money?

And where can I fill out this document on line? Or does it
have to be sent in because of the need for a signature?

And do I have to have that done before Jan. 1, 2009 to be
counted on my 2008 tax.
