To get a copy of the disbarrment order (and per curiam/pc or
any other info - if it exists) for an attorney :

Send the disbarment order request to the : Attorney Records Department
Appellate Division First Department - NY Court System 41 Madison Ave
26th floor New York, NY 10010 (646) 386-5893

include : 1) Attorney name/registration ID
2) Check for $3.00 made out to the "Clerk - Appellate Division"
3) A self-addressed stamped envelope

You should call their office to be sure the attorney you are making the
request was disbared in the First Department - Appellate Division . They
also may provide lists, etc. if requested and/or direct you to another
Department depending on where the attorney was disbarred. Such attorney
info does not appear to be online, so either via snail mail and/or in person
can disbarment info be obtained. -- Matt 2
