by the way, purpose of hearing. (question of treatment)
question of authorization for treatment/tests. and question of
temporary rates.
evidence to be produced:
by claimant: produce up to date medical
by employer or carrier: payroll of claimant, medical reports,
produce c-11, produce reimbursement request.(so doesn't look
that bad for this hearing)
