Post: Restoring Rights to own/carry firearms

I was convicted of Domestic Violence about 5 years ago and
would like to get my rights to own and carry firearms
restored. I went to the county clerks office at the local
court house and was informed that the forms I need to fill
out are online, but they weren't able to tell me the name
of the forms for liability reasons. How do I go about
restoring my rights?
