Post: Need advice for FOID appeal

I need to appeal the circuit court in my county to try to
get my FOID which was denied due to a felony 1 conviction
i had when i was 18 in 1992. attorney fees are too
expensive, so i need to do it myself. I know what I need
to do, just don't know how to go about doing it.

1. How do I initially petition the State's Attorney or
Circuit Court ? What form should this petition be in and
what should it contain ?

2. I want to sign an afferdavit stating that I have not
been in any trouble since then. How would I go about doing
that ?
